Thank you for contacting the Dermatology Center at Old Bridge. Please complete the new patient forms and bring with you to your office visit. Also, be sure to bring your insurance card and a referral (if required). Referrals must be submitted on the appropriate referral forms at the time of visit. If you have any questions as to whether you need a referral, please reference your health insurance card or contact your provider. Your primary care physician can fax over any referrals PRIOR to your appointment at 732-591-1772. Patients seen without the appropriate referral documents will be billed directly for the office visit.

We are located at 3663 Route 9 North, Suite 103 in the Renaissance Properties building in the Renaissance Plaza, located between Spring Valley and Ferry Road. Get more directions and online maps.
